研究射频热凝术结合高能量激光治疗盘源性腰痛的临床疗效。
选取江苏大学附属医院疼痛科2015年8月至2018年6月收治的30例椎间盘源性腰痛患者为研究对象,其中男性患者19例,女性患者11例;年龄45~70岁,平均(50±1.1)岁。所有患者行射频热凝术并结合术后高能量激光持续治疗1周,观察并比较治疗前、联合治疗后第1周、第1、3、6个月的视觉模拟评分(VAS)。
所有患者射频热凝手术和术后顺利接受高能量激光治疗持续1周均成功。28例患者治疗前VAS为(6.55±0.22)分,治疗后1周为(2.55±0.43)分,治疗后1个月为(2.01±0.35)分,治疗后3个月为(1.75±0.72)分,治疗后6个月为(1.05±0.02)分,术后VAS评分与术前评估比较,差异有统计学意义(P<0.05)。术后1~3月随访所有患者疼痛均已基本消失,随访6个月,除2例腰痛复发以外,其余均无复发,优良率为90%。

To study the clinical efficacy of radiofrequency thermocoagulation combined with high-energy laser in the treatment of discogenic lower back pain.
30 patients with discogenic low back pain who had been clinically diagnosed and admitted from August 2015 to June 2018 were selected as research objects. including 19 male patients and 11 female patients, aged 45-70 years with an average age of (50±1.1) years.All patients underwent radiofrequency thermocoagulation combined with postoperative high-energy laser therapy for 1 week. The pain was observed and compared before and after treatment(VAS score).
Radiofrequency thermocoagulation was successfully performed in 30 patients, and high-energy laser treatment was successfully performed for 1 week. VAS of 28 patients before treatment was (6.55±0.22)points, (2.55±0.43) points in 1 week, (2.01±0.35)points in 1 month, (1.75±0.72) points in 3 months, and (1.05±0.02)points in 6 months after treatment. All the patients had no recurrence except 2 cases of back pain recurrence, with excellent rate of 90%. There was a statistically significant difference between postoperative VAS score and preoperative evaluation (P<0.05).
Radiofrequency thermocoagulation combined with high-energy laser therapy is effective in the treatment of discogenic low back pain.
