目的

比较重症与非重症原发免疫性血小板减少症(ITP)患者及不同分期的ITP患者、合并其他自身免疫病的ITP患者外周血淋巴细胞计数、中性粒细胞/淋巴细胞比率(NLR)、血小板/淋巴细胞比率(PLR)的差异以及与生化指标的相关性分析。

方法

无菌采集47例ITP患者其中重症12例抗凝及促凝外周静脉血各2mL,检测中性粒细胞、淋巴细胞及血小板计数,计算NLR和PLR,并检测乳酸脱氢酶、白蛋白、球蛋白、胆固醇、甘油三酯,低密度脂蛋白、C反应蛋白指标。

结果

重症ITP患者外周血中性粒细胞计数低于非重症ITP患者[(4.43±2.09)×109/L比(6.91±3.63)×109/L,P<0.05];新诊断ITP、持续性ITP,慢性ITP、合并其他自身免疫性疾病的ITP四组患者的淋巴细胞计数差异有统计学意义(χ2=9.22,P<0.05);ITP患者外周血淋巴细胞计数与白蛋白呈负相关(r=-0.728,P<0.05),PLR与乳酸脱氢酶呈负相关(r=-0.415,P<0.05)。

结论

重症和不同分期ITP患者外周血淋巴细胞计数、中性粒细胞计数可能是其预后判断的指标之一。

Objective

To investigate the differences of peripheral blood lymphocyte count, NLR, PLR and their correlation with biochemical indexes in severe immune thrombocytopenia (ITP) and different stages of ITP.

Methods

A total of 47 ITP patients was enrolled in this study, including 12 severe cases. Each 2 ml of anticoagulant and procoagulant peripheral venous blood was collected, neutrophils, lymphocytes and platelets, and LDH, Alb, Glb, TC, TG, LDL-C, CRP were detected, and NLR and PLR were calculated.

Results

Neutrophil count in peripheral blood of patients with severe ITP was lower than that of non-severe ITP patients [(4.43±2.09)×109/L vs. (6.91±3.63)×109/L, P<0.05]; there were significant differences in lymphocyte count among the four groups of patients with newly diagnosed ITP, persistent ITP, chronic ITP and ITP patients with other autoimmune diseases(χ2=9.22, P<0.05). Peripheral blood lymphocyte count was negatively correlated with albumin(r=-0.728, P<0.05) and PLR was negatively correlated with LDH in ITP patients(r=-0.415, P<0.05).

Conclusion

Peripheral blood lymphocyte count and neutrophil count may be one of the prognostic indicators of severe ITP patients and their different stages.
