目的

探讨急性ST段抬高型心肌梗死(STEMI)患者心电图QRS波延长与心肌损伤之间的相关性。

方法

采用回顾性病例对照研究分析2017年2月至2019年11月经确诊为冠心病患者90例,根据QRS波长是否延长分为延长组(n=52)和非延长组(n=38),分析QRS延长与临床特征的关系,比较延长组和非延长组患者的心肌损伤指标CTnI、CK-MB、Myo及两组患者心脏功能LVEF、LVFS、LVEDd水平,分析延长时间与心肌损伤相关指标的相关性。

结果

QRS延长组血清CTnI、CK-MB和Myo指标均高于非延长组,差异有统计学意义(P<0.05),QRS延长组LVEF和LVFS的水平低于非延长组,LVEDd高于非延长组,差异有统计学意义(P<0.05);QRS延长水平与血清CTnI、CK-MB和Myo指标呈正相关(P<0.05),与LVEF和LVFS的水平呈负相关(P<0.05),与LVEDd水平呈正相关(P<0.05)。

结论

STEMI患者心电图QRS延长与心肌损伤程度呈正相关,监测ECG的QRS波群有助于评估STEMI患者的心肌损伤程度。

Objective

To investigate the relationship between QRS wave lengthening and myocardial injury in patients with acute ST-segment elevation myocardial infarction (STEMI).

Methods

Retrospective analysis of patients diagnosed with coronary artery disease from February 2017 to November 2019 was conducted and a total of 90 cases were involved in this study. According to the QRS whether the wavelength was prolonged or not, the patiants were divided into prolonged (n=52) and non-prolonged (n=38) groups, and the QRS prolongation and clinical characteristics of the relationship were analyzed, and myocardial injury index CTnI, CK-MB and Myo were detected. Levels of cardiac function LVEF, LVFS and LVEDd were analyzed for correlation between prolonged time and indices related to myocardial injury.

Results

Serum CTnI, CK-MB and Myo indexes were significantly higher in the QRS prolonged group than that in the non-prolonged group, and the differences were significant Statistically significant (P<0.05); the levels of LVEF and LVFS in the QRS extension group were significantly lower than those in the non-prolonged group; LVEDd was significantly higher than in the non-prolonged group, and the difference was statistically significant (P<0.05); QRS prolongation levels was significantly positively correlated (P<0.05) with serum CTnI, CK-MB, and Myo indexes, and there was a significantly negative correlation (P<0.05) between the levels of LVEF and LVFS, and a significantly positive correlation with the level of LVEDd (P<0.05).

Conclusion

ECG QRS prolongation is positively correlated with the degree of myocardial damage in STEMI patients, and monitoring the QRS wave forms of the ECG helps to assess the extent of myocardial damage in STEMI patients.
