Clinical features of stroke-associated pneumonia in the elderly patients

Abstract:

Objective

To analyze the clinical characteristics of elderly patients of acute stroke complicated with pulmonary infection.

Methods

963 cases of elderly stroke patients were retrospectively analyzed, which were enrolled in The third Affiliated Hospital of Southern Medical University from January 2009 to October 2015. According to the diagnosis and treatment of stroke associated pneumonia Chinese expert consensus, 223 cases of SAP patients were enrolled and divided into early SAP group and late SAP group according to fever time for hospitalization for 48 h. The clinical characteristics of patients with SAP were analyzed.

Results

The hospitalization time of early SAP group was (25±8) d and hospitalization time of late SAP group was (10±6) d. The difference was statically significant. Mortality rates of two groups was 35.4% and 10.0%, respectively. The difference of the mortality rate was statically significant. Klebsiella pneumoniae (18%), Staphylococcus aureus (12%), Acinetobacter (10%), Escherichia coli (5%), Pseudomonas aeruginosa (6%) were the major pathogenic bacteria.

Conclusion

Occurrence of acute cerebral stroke elderly associated pneumonia is closely related to many factors (choking cough, disturbance of consciousness, hypertension, diabetes, pulmonary heart disease, primary disease).
